REPORT AN INCIDENT
While this page is designed to provide guidance and clarification during the incident report process, it’s most important that you report your concerns through any of the three forms named below. The incidents reported through these forms are reviewed by the Threat Assessment & Behavioral Intervention (TABI) Team and are responded to as appropriate.
- Faculty should raise academic success flags (e.g., attendance or academic performance concerns, tutoring or disability referrals, etc.) through Starfish, located in the Bruin Portal.
- A link to the Academic Integrity Incident Report Form is available to faculty only and can be found in the Bruin Portal.

|What Gets Reported||
Alleged violations of the Student Code of Conduct, facial covering concerns, general conduct and/or campus safety concern.
Think: theft, bullying, threats and/or violence, weapons, drugs and alcohol, vandalism, gambling, etc.
Incidents involving sexual misconduct that may be in violation of the Title IX Policy.
Think: inappropriate touching, sexual harassment, dating violence, etc.
Situations where student(s) are in distress (e.g., student welfare).
Think: homelessness, emotional distress, changes in mood or behavior, disturbed writing, etc.
